Dr. Mohammed Safwat is a consultant orthopaedic surgeon whose practice centres on surgery of the hand, wrist and upper limb, including microsurgical repair and reconstruction.

Hand surgery sits at an unusual intersection. The structures are small and unforgiving — nerves and vessels measured in millimetres, tendons that must glide freely to work at all. But the consequences are not small. A hand that does not function properly changes how a person earns a living, holds their child, and sleeps at night.

Diagnosis before intervention

Most hand problems can be identified from a careful history and examination. Investigations are used to answer a specific question, not to compensate for an incomplete assessment.

An honest account of what surgery can achieve

Some conditions resolve without an operation. Some improve substantially with one. A few will not fully recover whatever is done — long-standing nerve compression is the clearest example. Patients are told which of these applies to them before a decision is made, not afterwards.

The operation is half the treatment

After tendon repair, nerve surgery and complex fracture fixation, the rehabilitation programme determines the result as much as the surgery does. Therapy is planned from the outset and its importance explained clearly.

Knowing when not to operate

Declining to operate is a clinical decision like any other. Where the risks outweigh the likely benefit, or where a non-surgical approach will achieve the same outcome, that is what is recommended.

Member of the Egyptian Society for Surgery of the Hand and Microsurgery (ESSHM), the national body for the specialty in Egypt.

ESSHM was established in 2007 and is a member of the International Federation of Societies for Surgery of the Hand (IFSSH) and the Pan African Federation of Societies for Surgery of the Hand (PAFSSH).
